Son of Peter and Joanna Kozlowski. Died of pneumonia at Camp Sherman Hospital, Chillicothe, Ohio during the Spanish influenza epidemic in Ohio. Nearly 1200 soldiers from the camp died from this outbreak. Camp Sherman was known as "Ohio's World War I Soldier Factory."
