Howard M.Jack 31 Mar.1880-6 Feb.1951 Catherine C.Jack 1875-1951 The death certificate shows that his middle initial as 'J' but the headstone says 'M'. Howard was born in Pennsylvania and his parents are Thomas Jack & Nettie Tatlinger. The information on the death certificate was provided by Catherine C.Jack.
