Note: stone is sunk and quite weather-worn. Difficult to locate and transcribe. Name on stone is spelled MIKOLAJEK however death was recorded in all records researched (Calvary burial record, St. Adalbert's Parish burial record, and state of Ohio death abstract) as MIKOLAJCZYK.
